A new version of a container started but did not answer 2xx on its
health_path within two minutes, so it never got traffic: the previous version
keeps running. Most often the server listens on another port than port (it
also gets PORT), listens on 127.0.0.1 instead of 0.0.0.0, or crashes at start.
get_container_logs shows what happened. A restart_container that does not
become healthy gives the same error.
